Early Intervention Service Coordinator

Program: Early Intervention (Birth–Age 3)
Employment Type: Part-Time / Full-Time / Independent Contractor
Reports To: Early Intervention Program Director

Angel Community Alliance, Inc. (ACA) is seeking a compassionate, organized, and dependable Early Intervention Service Coordinator to support infants and toddlers from birth to age three and their families. The Service Coordinator assists families in navigating the New York State Early Intervention Program, coordinating evaluations and services, developing and monitoring the Individualized Family Service Plan (IFSP), and connecting families with appropriate resources and supports.

Responsibilities:

  • Provide Initial Service Coordination (ISC) and/or Ongoing Service Coordination (OSC).
  • Assist families in understanding the Early Intervention process, available services, and their rights.
  • Coordinate evaluations, assessments, IFSP meetings, and authorized services.
  • Participate in the development, review, implementation, and monitoring of the IFSP.
  • Assist families with identifying and selecting appropriate Early Intervention providers.
  • Maintain communication with families, evaluators, therapists, providers, and other members of the child’s Early Intervention team.
  • Monitor service delivery and follow up regarding service delays, missed services, or family concerns.
  • Assist families with accessing community resources and additional supports.
  • Assist with transition planning as children approach age three.
  • Complete required Service Coordination case notes, contacts, documentation, and follow-up activities within required timeframes.

Requirements:

  • Bachelor’s degree or higher in Psychology, Social Work, Human Services, Early Childhood Education, Special Education, Sociology, Counseling, or a related field.
  • Must meet applicable New York State Early Intervention Program requirements for Service Coordination.
  • 2-3 years experience working with infants, toddlers, children, and families preferred.
  • Knowledge of and experience using the NYS EI-Hub is strongly preferred.
  • Strong written and verbal communication skills.

Angel Community Alliance, Inc. (ACA) is seeking a compassionate, organized, and experienced NHTD/TBI Waiver Service Coordinator to assist individuals with disabilities, traumatic brain injuries, and other complex needs in accessing and maintaining community-based services. The Service Coordinator is responsible for coordinating waiver services, developing person-centered service plans, monitoring service delivery, advocating for participant needs, and helping participants remain safely and independently in their homes and communities.

Responsibilities:

  • Provide Service Coordination in accordance with applicable NHTD and TBI Medicaid Waiver requirements.
  • Complete participant intake, assessments, referrals, and ongoing Service Coordination activities.
  • Develop and coordinate Initial Service Plans (ISP), Revised Service Plans (RSP), and Annual RSPs.
  • Coordinate with the Regional Resource Development Center (RRDC) regarding enrollment, service plans, approvals, and changes in services.
  • Maintain regular contact with participants, families, caregivers, waiver providers, and medical professionals.
  • Monitor whether approved waiver services are being delivered according to the participant’s service plan.
  • Coordinate services such as HCSS, ILST, PBIS, Community Integration Counseling, Structured Day Program services, and Peer Mentoring.
  • Advocate for participant choice, independence, dignity, community integration, and person-centered services.
  • Complete required monthly contacts, progress notes, service coordination notes, and supporting documentation.

Requirements:

  • Bachelor’s degree or higher in Social Work, Psychology, Human Services, Sociology, Counseling, Rehabilitation, Nursing, or a related field.
  • Must meet all applicable NHTD/TBI Waiver Service Coordinator qualification requirements.
  • Previous experience in NHTD/TBI Service Coordination, case management, Medicaid Waiver services, or community-based services preferred.
  • Prior NHTD and/or TBI Waiver Service Coordination experience; 1-2 years experience developing ISP, RSP, and Annual RSP service plans preferred.
  • Experience working directly with an RRDC preferred.

(ACA) is seeking a compassionate and dependable Peer Mentor to provide person-centered support to participants enrolled in the Nursing Home Transition and Diversion (NHTD) Medicaid Waiver Programs. The Peer Mentor uses appropriate lived experience, guidance, encouragement, and practical support to help participants increase independence, strengthen self-advocacy skills, develop community connections, and work toward their personal goals.

Responsibilities:

  • Provide Peer Mentoring services in accordance with the participant’s approved service plan.
  • Establish a supportive, respectful, and professional peer relationship with participants.
  • Use relevant lived experience appropriately to provide encouragement, guidance, and support.
  • Help participants develop self-advocacy, decision-making, problem-solving, and independent living skills.
  • Encourage participants to identify and work toward personal goals.
  • Support participants with adjusting to living independently in the community.
  • Assist participants with identifying community resources, activities, and natural supports.
  • Encourage community participation and social inclusion.
  • Immediately report health, safety, abuse, neglect, exploitation, or other significant concerns.
  • Complete required service notes, progress documentation, and other records accurately and on time.

Requirements:

  • Must meet applicable NHTD/TBI Waiver Peer Mentoring qualifications and program requirements.
  • Relevant lived experience that allows appropriate peer-based support and mentorship.
  • Understanding of challenges individuals may experience while living with a disability, TBI, or transitioning into community-based living.
  • Ability to use personal experiences appropriately while maintaining professional boundaries.
  • Strong communication, interpersonal, listening, and relationship-building skills.
  • Reliable, professional, and able to work independently.
  • Experience working with individuals receiving NHTD/TBI Waiver services preferred.

(ACA) is seeking an experienced and compassionate Positive Behavioral Interventions and Supports (PBIS) Specialist to provide behavioral support services to participants enrolled in the NHTD and TBI Medicaid Waiver Programs. The PBIS Specialist works collaboratively with participants, families, caregivers, Service Coordinators, and other team members to identify behavioral needs and develop person-centered strategies that promote independence, safety, community integration, and improved quality of life.

Responsibilities:

  • Conduct behavioral observations and assessments to identify behavioral concerns, patterns, triggers, and strengths.
  • Develop individualized Positive Behavioral Intervention and Support Plans based on participant needs, preferences, and goals.
  • Establish measurable behavioral goals and intervention strategies.
  • Teach participants appropriate coping, communication, self-regulation, and social skills.
  • Provide education, training, and guidance to family members, caregivers, and direct support staff.
  • Collect, review, and analyze behavioral data to monitor participant progress.
  • Participate in participant team meetings, service plan reviews, and interdisciplinary case conferences.
  • Complete required assessments, behavioral plans, progress notes, and monthly documentation on time.

Requirements:

  • Master’s degree or other qualifying education in Psychology, Applied Behavior Analysis (ABA), Social Work, Counseling, Special Education, Rehabilitation Counseling, or a related discipline.
  • Experience providing behavioral assessment, intervention, counseling, or behavioral support services to individuals with disabilities, TBI, or complex behavioral needs.
  • Knowledge of positive behavioral supports, behavior assessment, data collection, and person-centered planning.
  • Strong clinical observation, assessment, documentation, and communication skills.
  • Previous experience providing PBIS services within NHTD and/or TBI Waiver Programs preferred.

Angel Community Alliance, Inc. (ACA) is seeking a compassionate, organized, and experienced Independent Living Skills Training (ILST) Specialist to provide person-centered services to participants enrolled in the NHTD and TBI Medicaid Waiver Programs. The ILST Specialist works with participants to develop, restore, maintain, and improve independent living skills necessary for successful community living.

Responsibilities:

  • Conduct assessments and observations to identify the participant’s current level of functioning, strengths, and needs.
  • Develop individualized ILST goals, strategies, and training activities.
  • Provide hands-on teaching, coaching, modeling, prompting, and skill-building support in areas such as personal organization, household management, meal planning, budgeting, transportation, and community navigation.
  • Teach compensatory strategies for cognitive, memory, organizational, or executive-functioning challenges.
  • Promote participant choice, independence, self-direction, dignity, and community integration.
  • Monitor participant progress toward established ILST goals and modify training strategies as needed.
  • Collaborate with the participant’s Service Coordinator, PBIS provider, family members, and other waiver providers.
  • Complete required ILST assessments, plans, progress notes, and monthly documentation on time.

Requirements:

  • Bachelor’s degree or higher in Psychology, Social Work, Human Services, Rehabilitation, Occupational Therapy, Special Education, Counseling, or a related field.
  • Experience working with individuals with traumatic brain injuries, physical disabilities, neurological conditions, or developmental disabilities preferred.
  • Knowledge of independent living skills, rehabilitation, and community-based supports.
  • Strong teaching, coaching, communication, and interpersonal skills.
  • Previous experience providing ILST services within NHTD and/or TBI Waiver Programs preferred.
