Traumatic Brain Injury (TBI) Services
1. Service Coordination
We create individualized service plans that connect participants to Medicaid and
community supports.
- For those transitioning from nursing homes: we support safe return.
- For those in the community: we promote stability and prevent institutionalization.
Initial Service Coordination
Supports applicants with eligibility and paperwork.
Ongoing Service Coordination
Manages service delivery, communication, and monthly in-person check-ins.
2. Independent Living Skills Training (ILST)
Helps participants regain or maintain the skills needed for daily independence.
- One-on-one training in the home or community
- Supports those with disabilities or progressive conditions
- Group training available with RRDC approval
3. Positive Behavioral Interventions and Supports (PBIS)
Addresses significant behavioral issues with:
- Full behavior/environmental assessment
- Personalized behavioral plans
- Family and caregiver training
- Data tracking and plan reviews
